Emergency Care

The Emergency Room at Bertrand Chaffee Hospital is located on the first floor of the hospital and may be accessed through the main door of the hospital on the west side of the building or through the ambulance entrance. After 11:00 p.m., all patients must use the ambulance entrance.

Admission

Upon arrival to the Emergency Care Unit the following will take place:

  • A Registered Nurse will take you to a patient care area for an initial evaluation.
  • The RN will ask you about the problem that has brought you to the ER as well as past medical problems.
  • You will be asked about medications you are taking and any allergies you have.
  • Your vital signs (pulse, temperature, blood pressure) will be checked. Your condition will also be assessed by looking at wounds and lacerations, listening to your heart and other procedures.
  • You or a family member will provide the secretary with information including your address, phone number and insurance information.
  • Patients will be seen in order of the severity of illness, not necessarily in the order of arrival. While in the Emergency Room.

All patients will be seen by a doctor who will determine the appropriate care that is necessary. The doctor may order blood work, x-rays, EKG’s, breathing treatments, IV’s, oxygen or medications. The RN on duty will continue to assess each patient’s condition. If a patient or family member has any questions about the care that is being provided they should ask the nursing staff immediately. Patients should not eat or drink while in the emergency room unless directed to do so by the doctor or nursing staff.

Policies and Procedures
  • Visitors: Since space is limited in the Emergency Room, patients may have no more than two visitors at a time.
  • Phone Calls: Pay phones are available in the hallway off the lobby to contact family and friends. To ensure patient confidentiality, the staff will not discuss patients’ conditions over the phone.
  • Smoking: Smoking is prohibited throughout the hospital.
  • Phone Advice: The ER does not attempt to diagnose or offer treatment suggestions over the phone. You will be told to call your family doctor or come to the ER if you feel you need to see a doctor.
  • Discharge: Upon discharge an RN will review instructions with you and/or a family member and a written copy of the instructions will be provided for you to take home.

CARDIAC CARE UNIT

Bertrand Chaffee’s Cardiac Care Unit is located on the second floor of the hospital and offers intensive nursing and medical care to meet the needs of acutely ill patients. Unit staff are all Registered Nurses who have special education and additional training in the care of such patients.

What you need to know

The staff at BCH understands that having a loved one in CCU is stressful. To make sure that each patient receives the highest level of nursing care possible, the Unit operates differently than other parts of the facility.

  • Visiting Hours / Rules – Visiting is restricted to immediate family only and a maximum of two (2) persons at one time. Visiting hours for the CCU are 11 a.m. – 8:30 p.m. with no visiting from 1-2 p.m. and 5:45-6:45 p.m. You may be asked to limit your visit to a few minutes, or not visit at all depending on the patient’s condition.
  • Waiting Room - A waiting room is available for families on the second floor outside the CCU. All visitors must use the call system in the waiting room BEFORE entering the CCU.
  • Phone Calls – We ask that only one person in the family be designated to call the CCU to check on a patient’s condition and then relay the information to the rest of the family. The name of the designated person should be given to the staff at the time of admission. Calls are accepted any time (night or day).
  • Special Equipment – The CCU is equipped with cardiac monitors to allow nurses to see the patient’s heart rhythm even when they are not in the room. IVs and related devices, oxygen, ventilators, and pacemakers are all part of the CCU environment. Because of all this equipment, no personal radios or televisions are allowed.
  • Smoking is strictly prohibited.
  • Diet – Most patients are on a special diet. Please do not bring any food or drinks without checking first with the nurse.

Most routine services performed on site. Esoteric tests are sent to a reference laboratory with results generally available in 24-48 hrs.

Physician Requests

All physician requests, written or faxed, are accepted (local and non-local) provided it is less than 6 months old, legible, and the diagnosis is present. Phone requests are accepted from physician's offices if they are followed up within 48 hours with a written or faxed request.

Insurance

All Insurance is accepted except Community Blue.

Community Blue “STATS” or pre-operative testing is accepted.

Respiratory Care Services

Respiratory Care is the treatment and care of people that have serious problems associated with the lungs and breathing. Respiratory Care also includes diagnostic evaluation to find the causes of the problem and how to care for it.

We are pleased to have recently acquired a new Pulmonary Function machine that allows expanded diagnostic testing. This new equipment now allows us to help the physician diagnose and treat different lung conditions and diseases. The different tests include:

  • Pre/post bronchodilator – Useful in determining a reactive airway response to medications such as Albuterol metered-dose inhalers.
  • Nitrogen Washout – enables the physician to determine different lung volumes in patients.
  • DLCO – this test will determine how well oxygen can diffuse across the membranes between your lung and your bloodstream.

The Pulmonary Function Lab is open for appointments Monday – Friday from 8:00 a.m. – 4:00 p.m. Tests must be ordered by a physician.

Cardiac Rehabilitation Program

  • Exercise Sessions: MWF 7:00 a.m. – 1:00 p.m.
  • Covered by most insurances for cardiac conditions of post myocardial infarction, post coronary bypass surgery or chronic angina
  • Angioplasty, heart valve surgery & stents are now covered
  • Requires a stress test for participation
  • Includes monitored exercise and education regarding heart function, risk factor management and guidelines for ongoing personal exercise program

The health professionals at Bertrand Chaffee Hospital are continually working to care for you and your heart. A healthier lifestyle can be enhanced through Cardiac Rehabilitation.

As part of the Cardiac Rehabilitation team, we will help you, the Cardiovascular patient, improve your heart’s health through a program of exercise and health education on both an inpatient and outpatient level.

Participants in this program are those eligible patients who seek to improve their cardiovascular function and physical fitness. Our patients are those who have either an acute myocardial infarction – “MI” (heart attack), have had coronary artery bypass graft surgery or other cardiac surgery, or are at high risk for coronary artery disease.

Inpatient Care

Cardiac Rehabilitation begins as early as possible in the recovery process usually 204 days after MI or as soon as the patient’s condition permits. The timing is designed to help prevent the “deconditioning” effects of prolonged bed rest and relieve anxiety as well as depression.

A specific exercise and activity schedule is designed for every patient with the progress determined by each individual’s cardiovascular response.

The first step begins at the bedside. The early phase calls for mild calisthenics and range of motion exercises, progressing to mild resistive exercise, walking and mild stair climbing. Throughout this period, an educational program is underway, including discussion on developing an understanding of the nature of each individual’s heart problem and treatment. Counseling on medications, diet, and risk factor reduction will also take place.

Outpatient Care

At this point, The Bertrand Chaffee Hospital Cardiac Rehabilitation Department becomes a place to visit three (3) times a week over a period of twelve (12) weeks.

Newcomers will be allowed to enter the program at this stage. A physician’s referral is required, as well as an exercise stress test.

Each exercise sessions includes:

- warm up exercises

- workout on such equipment as stationary bike, arm ergometer, wall pulleys, treadmill, rowing machine and other devices.

During this session, electrocardiogram monitoring and blood pressure measurements are used to determine your cardiovascular responses. The team of the Cardiac Rehab Department as well as the physician director will evaluate this information and make subsequent plans for future exercise sessions. Records of performance and cardiovascular responses will be sent to your physician.

Patients will also be encouraged to follow a detailed home exercise program. In addition, health education programs are offered during your rehabilitation session.

Requirements

Physician Referral – when a referral is received, we will contact your physician for results of an exercise stress test and recent blood work. If your physician would prefer, a stress test can be arranged for you through the Cardiac Rehabilitation Department.

Medical and nursing history and a physical will be performed by the Cardiac Rehabilitation Team upon entry to program.

You as the patient can call at anytime for information on your cardiac care and recovery.

Classes

The air conditioned exercise facilities are located in the basement of Bertrand Chaffee Hospital, 224 East Main Street, Springville, NY.

Classes are offered Monday, Wednesday and Friday's.

Financial Facts

These costs are covered by most insurance plans, including Medicare, at about 80% reimbursement.

Blue Shield and Medicare payments are billed and collected by the hospital. Commercial insurance forms should be brought to the business office at Bertrand Chaffee Hospital.

If you have any questions about your billing, please call the business office at (716) 592-2871.

Insurance plans do not always provide full coverage. Your bill is a contract between you and your insurance company. The hospital will cooperate to the fullest in expediting your claim

Patient and Family Services

The Department is directed by a registered nurse. Discharge planning is done by experienced and caring discharge planners, under the supervision of the RN supervisors.

We are located on the first floor of the Hospital. Our general phone number is 592-2871 ext. 213. Our fax number is 592-2871 ext. 214.

We are available Monday through Friday, 8:00 AM to 4:00 PM.

Mission

Bertrand Chaffee Hospital endeavors to insure the total well-being of patients entrusted to their care. During the course of medical care for acute and chronic illnesses, some patients/families experience periods of high vulnerability and stress during transition from in-patient to out-patient/extended care status.

Role

The principal roles of the Department are those of counseling and discharge planning. The staff assessess and shares knowledge with medical and nursing staff to plan for patient care and patient’s discharge needs. The Department assumes a major responsibility for discharge planning to patients who need community services to function in their own homes and who need alternate placement in an adult care facility, nursing home or rehabilitation unit.
